I have been working out with a squat machine at gym, and have developed this ugly roundness to my upper back where shoulders meet. I hate it. My kids call me the incredible hulk, not what I am trying to achieve when I am using this machine. I use it to workout my legs but it looks like it is also building muscles on my upper back. Hate it.
Is this possible?
I don't know of any other machine that can be doing this to my upper body.
Please advice.

Sounds more like a postural issue to me. Are you doing any exercises for the rest of your body? It is important to exercise all muscles of the body so you don't develop imbalances. Correct form when weight training is key. It might be wise to have a trainer watch you while you work out to check your form. Due to aging, gravity, and sitting, the shoulders naturally tend to round forward. The trainer can show you some specific exercises to strengthen your upper back and shoulders to correct the roundness.
Rolfing is a type of therapy that can correct posture issues. Great for fixing any chronic pain too.
